Job Title: Family Navigator

Location: Derby (Community Hub – Parcel Terrace)

Pay Rate: £13.62 per hour (Incl.Hols)

Contract Type: Temporary to Permanent

Hours: 28 hours per week

Working Pattern: Flexible working pattern including some evenings (up to 6:30 pm twice per week)

Shift Pattern: Monday to Friday (with flexibility required)

Start Date: ASAP

Driving Requirement: Full UK Driving Licence & Access to Own Vehicle (Essential)

About the Role

This is a meaningful and rewarding opportunity to join a community-based service as a Family Navigator on a temp-to-perm basis, supporting parents, carers, and families to improve wellbeing, stability, and life opportunities.

The role focuses on delivering holistic, person-centred support, helping individuals overcome barriers such as poverty, housing insecurity, and access to employment or education.

You will work both within a community hub environment and out in the community, building strong relationships with families and external services to create positive outcomes.

This role combines direct support, outreach work, and multi-agency collaboration, making it ideal for someone passionate about empowering families and strengthening communities.

Key Responsibilities – Family & Community Support

  • Provide one-to-one, group, and family-based support tailored to individual needs
  • Build positive, trusting relationships with parents, carers, and families
  • Support individuals to develop life skills, confidence, and independence
  • Provide advice, guidance, and signposting to relevant services
  • Facilitate group sessions, activities, and family engagement initiatives
  • Support individuals towards education, volunteering, and employment pathways
  • Deliver outreach support within homes and community settings
  • Maintain a safe, welcoming, and inclusive environment

Operational & Hub Responsibilities

  • Support the day-to-day running of the community hub
  • Assist with administrative tasks and service delivery
  • Maintain accurate case notes and records
  • Track outcomes and monitor progress of individuals and families
  • Contribute to reporting and service evaluation
  • Ensure all work is delivered in line with policies and procedures

Partnership & Multi-Agency Working

  • Build strong relationships with local services and community organisations
  • Work collaboratively with statutory and voluntary agencies
  • Encourage referrals and engagement with support services
  • Maintain up-to-date knowledge of local support pathways
  • Contribute to developing community opportunities and partnerships

Successful Applicant Profile

Essential

  • Experience supporting vulnerable individuals, families, or communities
  • Experience of multi-agency or partnership working
  • Strong communication and interpersonal skills
  • Ability to build rapport and maintain professional boundaries
  • Good organisational skills and ability to manage a caseload
  • Resilient, proactive, and solution-focused
  • Confident using IT systems and maintaining records
  • Full UK driving licence and access to a vehicle
  • Understanding of safeguarding principles

Desirable

  • Experience within community, family support, or voluntary sector settings
  • Knowledge of local services, housing, or welfare systems
  • Experience facilitating group sessions or activities
  • Understanding of trauma-informed or person-centred approaches
  • Relevant qualification (e.g. Level 3 in Health & Social Care or equivalent)

Safeguarding & Compliance

This role is subject to:

  • Enhanced DBS check
  • Two satisfactory references
  • Successful completion of the temporary period before permanent transition
