The post holder will provide specialist nursing expertise for individuals with diagnosed or suspected learning disabilities and autism within a remand prison setting.
The role focuses on identification, assessment and support for individuals with neurodevelopmental needs, ensuring appropriate reasonable adjustments are implemented to support engagement with healthcare and the prison regime.
The post holder will work with complex cases, undertaking specialist assessment of need within the prison environment and coordinating appropriate interventions to improve health outcomes and reduce health inequalities for people with learning disabilities and autism.
Main duties of the job
- Identify individuals with learning disabilities and autism through screening, referral and clinical assessment processes.
- Undertake comprehensive neurodevelopmental and learning disability assessments to identify health and support needs.
- Develop and implement person-centred care plans addressing health needs and environmental adjustments required within the prison setting. Additionally ensure that care plans are reviewed.
- Provide specialist nursing advice regarding the management of individuals with learning disabilities and autism.
- Support individuals to understand healthcare information through adapted communication and accessible materials.
- Work with patients presenting with complex needs, including co-occurring mental health, behavioural or communication difficulties.
- Ensure reasonable adjustments are implemented to support access to healthcare and engagement with prison services.
- Work to enhance the standards of nursing practice across the Service. Participate in local audit and trust-wide audits.
Job responsibilities
- Provide supervision and support to the Band 4 Neurodevelopmental Support Worker.
- Ensures effective delegation of tasks to other within the team to meet prioritised needs.
- Offer professional advice to members of the healthcare team regarding neurodevelopmental care.
- Contribute to reflective practice and clinical supervision within the team.
- Compiles reports and documentation as required by the service.
- Supports in the compilation of incidents and accident reports within agreed deadlines.
Qualifications
- RNLD
- RMN
- NMC 998 or equivalent
- MSc, working towards, or equivalent
