Clinical Lead Advanced Care Practitioner

About the Role
We are looking for an enthusiastic, forward‑thinking Clinical Lead Advanced Care Practitioner (ACP) to shape the future of community‑based acute care within our Acute Care at Home service.
Key Responsibilities
- Provide overall leadership and management responsibility for the clinical teams across the Acute Care at Home (ACAH) Service.
- Translate the ACAH vision and values into effective, empowered clinical teams that meet the needs of service users across Staffordshire.
- Maintain a high‑fidelity focus on Quality and Governance to ensure compliance with national, regional and local standards, policies and procedures.
- Act as a key stakeholder alongside Operational Teams in developing a flexible and sustainable workforce.
- Participate in the Senior Manager on‑call rota.
- Identify resource requirements and communicate effectively with staff to ensure appropriate utilisation, including contingency planning and adjustments.
- Be responsible for developing policy and procedure relating to Advanced Practice across ACAH.
- Propose, draft, interpret and implement policies and guidelines impacting the ACAH service.
- Support the Head of Operations with interpretation of national policy and standards, adapting and implementing relevant guidelines.
- Lead and promote compliance with best practice across the ACAH, developing mechanisms for shared learning and clinical networking.
- Lead and innovate the service vision for hospital admission avoidance through multiprofessional collaboration and a comprehensive understanding of services across Staffordshire.
- Direct expert clinical care, including advanced health assessment, initiation, formulation and alteration of treatment and monitoring of care programmes.
- Work autonomously as an expert practitioner within patients’ homes, providing acute assessment, intervention and medical management.
- Demonstrate advanced clinical skills and competencies to deliver safe acute care at home for all service users.
- Provide highly specialised professional advice, consultancy, clinical expertise and leadership to patients, carers and staff, working collaboratively with multidisciplinary teams.
- Communicate highly complex and sensitive information to patients, relatives and professionals, and using alternative methods when necessary.
- Ensure the highest standards of clinical care are consistently delivered and keep practice up to date through clinical supervision and CPD.
Qualifications and Experience
- MSc in Advanced Clinical Practice or equivalent (ePortfolio).
- Minimum five years’ experience as an Advanced Clinical Practitioner in Emergency, Acute or General Medicine.
- Independent non‑medical prescribing.
- Experience in developing and implementing advanced practice policies and procedures.
- Strong communication and leadership skills, with ability to influence and collaborate across multidisciplinary teams.
- Proven commitment to quality improvement and ongoing professional development.
- Positive mindset supporting organisational change, continuous learning and improvement.
- Accountability for delivering agreed objectives and maintaining high-quality work.
- Ability to travel to and work across multiple sites.
National and Professional Requirements
- Portfolio of certified/accredited professional development activity undertaken in Advanced Clinical Practice.
- Current NMC registration and compliance with NMC practice standards.
