South Warwickshire University NHS Foundation Trust is recruiting an experienced District Nurse to lead, inspire and shape the future of community nursing in Leamington Spa, Southam and surrounding villages. The role is based in both Leamington and Southam.
Serve as the clinical lead for all patients on the community nursing caseload. Provide advanced autonomous practice including diagnosing, prescribing, planning and evaluating care, using evidence‑based clinical guidelines. Conduct comprehensive assessments, physical examinations, and develop personalised care plans. Order investigations, interpret results, and manage medication prescribing in line with local and national policies.
Act as a resource for the Place‑Based Team and other community teams by providing teaching, education and clinical expertise. Promote research, innovation and evidence‑based practice, and contribute to quality improvement initiatives.
Oversee the safe use of clinical equipment, ensure all records are compliant with SWUF and NMC guidelines, and maintain competence in prescribing as an independent prescriber.
Provide highly specialised treatment, including leg ulcer care, end‑of‑life and wound care, continence management and drain management. Perform thorough history, systematic physical examinations, baseline data collection and formulate complex care plans for unpredictable situations.
Lead audit and monitoring of all care components, participate in personal development and support clinical competency development for team members.
Effectively manage the delivery and reception of complex or sensitive information, using persuasion, mediation and assurance techniques. Develop therapeutic relationships through active listening and reflection, support patients as active partners in care planning and maintain confidentiality across multidisciplinary teams.
Promote patient and family confidence in making informed decisions, support staff in clinical examinations and assessments, and develop the team’s knowledge on routine care pathways. Deliver educational sessions on supervision, quality improvement and learning‑from‑experience.
Contribute to the development, implementation and monitoring of clinical policies, procedures and guidelines. Provide clinical supervision and participate in the clinical appraisal process. Lead recruitment and selection, identify clinical development needs, and advocate for patients and the service.
