Overview
An exciting opportunity exists for a Clinical Nurse Specialist in Palliative Care to gain experience working in a community setting (1.0 WTE). The post holder will work predominantly in the community setting with flexibility to work within the whole of the Specialist Palliative Care service as required. This is a 7‑day service which runs from 08:00 to 20:00.
Responsibilities
- Provide specialist palliative care to patients with complex problems.
- Act as a specialist resource to generic health and social care professionals.
- Deliver education and training to develop palliative care practice.
- Conduct comprehensive and focused assessments, identifying subtle changes in condition and acting appropriately.
- Work in partnership with clinicians and health professionals to plan and implement treatment for palliative care and life‑limiting illness.
- Facilitate discussions with patients, families, and significant others concerning palliative care and end‑of‑life care plans, and refer to other services or professionals as appropriate.
- Work independently and as part of a multidisciplinary team, communicating effectively with a wide range of healthcare professionals.
Qualifications & Experience
- Substantial years of post‑registration experience, including palliative, cancer, or non‑cancer long‑term condition nursing.
- Demonstrated ability to perform a full holistic assessment and understand principles of symptom control.
- Understanding of specialist palliative care and the context of its delivery within a community setting.
- Evidence of ongoing professional development; experience of teaching desirable.
