Are you a qualified mental health nurse ready to make a real difference in people’s lives? As a Band 6 Care Coordinator within Southwark Community Mental Health Services, you will play a key role in supporting individuals on their journey to recovery, leading care plans, delivering targeted interventions, and helping service users overcome barriers.
You will manage your own caseload, collaborate with a multidisciplinary team, and develop recovery‑focused solutions tailored to each individual’s needs. You will empower service users to take ownership of their care while ensuring they have access to the resources and support they need to thrive.
The position is based in the Camberwell and Peckham Team at St Giles CMHT. The role will be primarily based at St Giles House, CMHT, with regular visits to community agencies and clients’ homes as required.
Key Responsibilities
- Co‑ordinate care, communicate with other professionals and ensure regular reviews are held.
- Lead and manage the work of others on the community team as required.
- Independently carry out assessment, care planning and care coordination with excellent interpersonal skills.
- Act as interface between partner agencies to facilitate timely and effective transition and discharge into the wider community.
- Maintain up‑to‑date documentation and relevant assessments on patient electronic records.
- Provide risk assessment and risk management, including reporting incidents and ensuring safe custody of medicines, sharps, and clinical equipment.
- Implement safeguarding procedures, Duty of Candour and infection control measures in a community setting.
- Administer medication according to NMC professional standards and manage medication side effects.
- Engage in practice reflection, clinical supervision and support junior staff.
- Demonstrate leadership skills within the clinical context and support a flexible approach to care provision with focus on choice and social inclusion.
- Promote recovery principles, encouraging service users to adopt self‑care and independence strategies.
Qualifications & Experience
- Clinical experience as a mental health nurse at Band 6 level or equivalent.
- Significant knowledge of Mental Health Law, including the Community Care Act 2015, Mental Capacity Act 2005 and Community Treatment Orders.
- Strong communication skills and ability to work collaboratively with multidisciplinary teams.
- Experience in case management, risk assessment, safeguarding and medication administration in a community setting.
- Leadership experience and the ability to support and supervise junior staff.
- Good IT skills with proficiency in electronic health record systems.
