About the role
We have an exciting opportunity to recruit a Mental Health Practitioner in Sutton Adult Community Services. The role is a permanent full‑time position, 37.5 hours a week, typically Monday to Friday, 9 am to 5 pm, with flexibility to be discussed at interview. The successful candidate will support patient transition between secondary and primary care, provide step‑down sessions, and collaborate with the General Practice Team and clinical supervisors from SWLSTG.
Responsibilities
- Provide medicine education, adherence advice and lifestyle support for patients and carers presenting with mental health problems.
- Assist with early intervention and shared decision‑making around self‑management.
- Support patients to stay well within their local communities, reinforcing newly learned psychological techniques.
- Support patients on the SMI register to access and complete annual physical health checks and perform physical health monitoring for complex needs.
- Promote the safety and well‑being of service users and their families/carers.
- Make referrals and signpost patients to local community services within NHS and VCSE partnerships.
- Carry out triage and comprehensive assessments as required.
- Work with patients and team members to support engagement with appropriate services and treatment, including onward referral to secondary mental health services or signposting to other mental health services, social prescribers, voluntary services, and community‑based support groups.
- Support transitions from secondary to primary care through up to three step‑down sessions at the point of transition, and support transitions from primary to secondary care when more intensive support is required.
- Support primary care teams with the administration of long‑acting injections for patients with complex needs.
- Develop close relationships with primary care practices, attending practice meetings and maintaining regular informal contact.
- Work closely and in partnership with colleagues within primary care and the Trust to ensure decisions provide the best care and a seamless pathway for service users.
