An exciting opportunity has arisen for a full‑time Band 7 Advanced Nurse Practitioner to join our South Birmingham Neighbourhood Mental Health Team, working with Hall Green PCN.

Responsibilities

  • You will be responsible for providing mental health triage, assessment and outcomes‑based intervention for service users, including nurse prescribing, within the Primary Care Mental Health Team.
  • You will be responsible for identifying need and the best course of advice, support or intervention.
  • You will be responsible for promoting psychosocial wellbeing by identifying and then supporting pathways for service users and carers.
  • Crucial to this role will be your ability to ensure robust communication across BSMHFT and primary care, in accordance with each individual service user’s needs, by providing support and advice to all colleagues with the PCN as required and by participating within the local neighbourhood MDTs.
