Mental Health Practitioner (Memory Assessment Services)
Band 5 Mental Health Practitioner position in the South Memory Assessment Service, Cambridge and Ely. The role is open to registered nurses with attributes to enhance a developing Memory Assessment Service.
Key responsibilities include contributing to the assessment of memory functioning, supporting post‑diagnostic care, working collaboratively with the multidisciplinary team, and providing support and education to peers, new staff, non‑registered staff and students.
- Work within the integrated care team‑mental health memory service to meet patient needs.
- Contribute to the assessment of memory functioning, health and social care needs, interpreting, planning and implementing treatment and care within professional guidelines, with the support of a Band 6 mental health practitioner.
- Avoid hospital admission where possible and reduce inpatient length of stay to ensure patients receive appropriate care in the most suitable setting.
- Keep the patient at the centre of care and ensure patient and carer participation in decision making.
- Support the Band 6 mental health practitioner with interventions, including guidance to people with dementia and their carers.
- Organise and manage own time, allocating work appropriately.
- Maintain high‑standard record keeping.
- Use evidence‑based practice to ensure clinical care is effective and appropriate.
- Highlight and report appropriate and potential risks.
- Contribute to the service/organisational aims of the Neighbourhood teams and the new model of care.
- Keep accurate and timely written records in compliance with Trust policy and NMC guidance.
- Inform and update all members of the multidisciplinary team, colleagues, service users and appropriate others of changes to nursing care plans, patient progress and other relevant matters.
- Provide support and education to peers, new staff, non‑registered staff and students.
- Support the Band 6 with day‑to‑day planning and delivery of a high‑quality nursing service.
Education / Qualifications
- RMN / RN / NMC Registered Nurse
- Evidence of professional development
- ENB 998 or other teaching qualification, mentorship
Experience
- Experience in acute or community mental health setting – can be part of training.
- Experience of working with people diagnosed with dementia – can be part of training.
- Community experience
- Primary care experience

- Ability to assess, plan and implement care.
- Recognition of limits of own authority within the role.
