Job Description:

We are seeking an enthusiastic and motivated Clinical Pharmacist to join our practice team.

This is an exciting opportunity to work in a patient-facing role within general practice, supporting medicines optimisation, improving patient outcomes, and contributing to high-quality care.

You will work closely with GPs, nurses, and the wider multidisciplinary team, delivering clinical services and supporting safe and effective prescribing.

Successful candidate will be employed directly through the practice. There is a possibility of some extended hour working (outside of our core practice hours)

Main duties of the job

  • Undertake structured medication reviews and manage polypharmacy
  • Support patients with long-term conditions
  • Provide expert medicines advice to clinicians and patients
  • Reconcile medicines following hospital discharge
  • Contribute to medicines safety and clinical governance
  • Support prescribing initiatives
  • Deliver education and training to practice staff
  • CPPE pathway is essential (or be willing to achieve with us)

Job responsibilities

The post holder will work within their professional boundaries as part of a multidisciplinary team, supporting general practice with medicines optimisation, patient care, and clinical services.

You will:

  • Work alongside pharmacists, GPs, and wider PCN teams
  • Be supported by a Senior Clinical Pharmacist and GP supervisor
  • Lead on medicines optimisation and safety
  • Deliver patient-facing clinics and medication reviews

Person Specification

Experience

  • Minimum 3 years post-qualification experience
  • Strong clinical and therapeutic knowledge
  • Understanding of general practice and primary care prescribing
  • Excellent communication and interpersonal skills
  • Ability to manage medicines optimisation issues
  • Ability to work autonomously and within a team
  • Experience in primary care
  • Familiarity with EMIS Web

Qualifications

  • Masters degree in Pharmacy (MPharm) or equivalent
  • Postgraduate diploma or equivalent experience
