Two senior band 6 mental health practitioners (occupational therapist or physiotherapist) are sought to join a dynamic team in the Community Therapy Service in the North and North East of Oxfordshire.
The role requires a senior clinician with a mental health background who is enthusiastic to develop their clinical practice within innovative and supportive multidisciplinary teams, treating patients in their own homes.
Provide high‑quality mental health rehabilitation to patients (adults) referred to or within the Community Therapy Service (CTS).
Help patients achieve rehabilitation goals and guide other CTS practitioners in supporting patients with mental‑health needs.
Support the community response physical health pathway and work closely with the Community Mental Health Team (CMHT) to minimise duplication, maximise efficiency and facilitate a seamless pathway between services.
Work closely with other multidisciplinary team members, social services and voluntary organisations for adults with a wide range of physical and mental health conditions.
