We are looking for a highly skilled and motivated Population Health Management (PHM) Operational Lead to join the South & West Hertfordshire Health and Care Partnership (HCP). This senior role is ideal for someone with strong analytical expertise, excellent relationship building skills, and a passion for using data to improve population health and reduce inequalities. As the PHM Operational Lead, you will play a pivotal role in developing PHM capability, embedding analytics into clinical and operational workflows, and supporting the creation of insight-driven system-wide transformation. You'll help position the HCP at the leading edge of PHM, shaping decision making and driving improvements in outcomes, value and care delivery. Main duties of the job
- Lead the development of operational PHM capability within neighbourhood teams.
- Build reporting infrastructure to measure outcomes.
- Produce insight packs, ad hoc analytical briefings and predictive modelling.
- Embed PHM tools into clinical and operational workflows.
- Provide analytical insight to inform resource allocation, financial modelling and business case development.
- Support the adoption and improvement of PHM tools.
- Work with partner analytics teams, including local authority public health, adult social care and children's services.
Qualifications and experience
- Advanced analytical and statistical skills, with experience in modelling, forecasting and visualisation.
- Strong experience working with clinicians and operational managers to influence decision making.
- Demonstrable experience in leading analytical work across complex systems.
- Excellent interpersonal, communication and influencing skills.
- Expertise in presenting complex information clearly to senior stakeholders.
- Values-driven, resilient and collaborative approach.
Job responsibilities
- Lead the development of PHM capability across the HCP, including training, mentoring and upskilling of colleagues.
- Develop and maintain reporting infrastructure for neighbourhood and place-based outcomes.
- Produce insight packs, analytical briefings and predictive modelling.
- Provide insights to inform decision-making on system resource allocation and commissioning options.
- Track and validate system-wide outcomes.
- Integrate risk stratification, predictive modelling and PHM tools into clinical and operational workflows.
- Broaden organisational understanding of PHM methodologies and promote best practice.
- Work with trust BIU teams to ensure PHM principles and tools are adopted and effectively utilised.
- Build strong relationships with system partners.
- Help build alignment and understanding across different organisational cultures, priorities and analytical needs.
Person Specification
Education and qualifications
- Educated to masters level or equivalent senior level experience in a specialist area.
- First degree in a mathematical or computer science based subject.
Knowledge
- In-depth knowledge of specialist population health management, including variation in healthcare and healthcare optimisation.
- Specialist knowledge in using data to demonstrate impact on population health outcomes.
- Advanced knowledge of statistical concepts and presentation of statistical analysis using visualisation tools.
Experience
- Experience of working in an expert capacity with clinicians and operational managers to inform and influence outcomes.
- Demonstrable experience in leading transformational change and quality improvement within the workforce.
- Extensive experience of interpreting highly complex information, distilling key messages and presenting options to senior internal and external audiences.
- Experience of using a modelling or simulation tool to produce predictive analytics.
- Experience of modelling acute and community provider demand and capacity, including bed modelling.
- Experience of analysing and integrating data from different provider settings (acute, community, mental health).
- Expert user of systems and experience of designing, adapting and managing information systems.
- Advanced MS SQL skills.
- Extensive experience of complex and successful NHS and non-NHS stakeholder management.
- Experience of designing and undertaking end-to-end analysis including data collection, interpretation and presentation of information.
- Experience using statistical analysis tools, e.g. R, Python, to analyse large data sets.
