Community Sister / Charge Nurse

Overview

Within the one team, we have Nursing, Physio and OT teams, each with their own Team Leads and, as a Community Sister/Charge Nurse, you will have the benefits of an enhanced scope of practice whilst championing the highest quality of nursing care.

Different to some teams, your priority will be direct patient assessment and treatment, with the Team Leads providing most of the team organisation and management, along with the support of our highly skilled administrators. You will be completing holistic assessments of patients at home, including identifying therapy needs and goals. You will case manage these patients yourself, giving you invaluable experience if you are looking to progress to a Community Matron role in the future. Another aspect is in-reach, where you are based at the hospital sites to proactively identify patients who can be discharged home with the team's support. This will be undertaken on a rota basis of likely one month at a time as consistency and relationships with internal and external stakeholders is essential. In addition to case managing your own patients and undertaking in-reach, you will support with the clinical delivery of complex nursing to patients on the broader team's caseload and responding to 2hr UCR referrals on behalf of the Community Nursing Teams. In return, we will support you with CPD opportunities including formal qualifications, e.g. DN, or vocational training opportunities.

Main duties of the job

High quality clinical management for patients with complex care needs, who are at risk of deteriorating health that may result in admission to hospital.

High quality clinical and interpersonal relationships to negotiate and support the discharge of patients from inpatient settings.

Using High-level Patient-centred Assessment Skills For The Health And Wellbeing Of Patients With Complex Needs, To Provide An Excellent Service In The Community Which Includes:

• First point of contact for a patient, co-ordinating their care, anticipating and dealing with problems before they lead to exacerbations including thorough both nursing and holistic assessments, care and timely onward referral when appropriate.
• Palliative Care: symptom management including assessment of acute symptom onset, care for the patient and their family/carers and in accordance with national guidance on end of life care.
• Public health: health protection and promotion programmes that improve and reduce inequalities.
• In Reach: discharge planning incorporating education to patients, relatives and carers to support their discharge from hospital, engagement with peers and colleagues to ensure safe discharge pathways and identification of appropriate patients who can be transferred from an inpatient setting to the community
• Caseload management: ensuring appropriate allocation and delegation of clinical tasks within the team to ensure safe care is delivered to the highest quality by the most appropriate person.

• BSc in Nursing Studies or equivalent.
• Practice assessor
• RNA Qualification
• NMC Registration

Desirable criteria

• District Nurse SPQ.

Experience

Essential criteria

• Must be a qualified nurse with relevant post qualifying experience.
• Experience in supporting patient and person-centred care.
• Experience in catheter care.
• Experience in administration of medication.
• Experience of administrating intravenous antibiotics and PICC/Midline care.
• Experience in end-of-life care, palliative care.
• Experience of multi-professional working.
