Summary

We have an opportunity for a Nurse, Occupational Therapist or Social Worker to join our well established Early Intervention in Psychosis team as a Care Coordinator. The role involves detailed initial assessment incorporating the CAARMS assessment to identify at‑risk mental states (ARMS) or first episode psychosis (FEP) and planning care over the next 3 years. The team uses a Recovery philosophy to collaboratively identify the individual’s goals and support them toward achieving these.

Responsibilities

  • Conduct initial assessments and formulate care plans.
  • Collaborate with clients and team members to identify and support personal recovery goals.
  • Support clients with housing, finances, vocational activities and social inclusion.
  • Manage risk using appropriate tools and maintain My Care Record.
  • Work independently and as part of the team, structuring workload and diary.

Qualifications & Skills

  • Significant experience in community care coordination.
  • Strong IT skills and working knowledge of My Care Record (previously CPA).
  • Positive risk‑taking approach and familiarity with risk tools such as the Steve Morgan tools.
  • Ability to drive and access a vehicle for business use.
