You can expect a manageable case load and a supportive management team to ensure you can work independently while support is readily available.
You'll be working alongside a multi-disciplinary team as the duty worker on our rota and will be regularly collaborating with external agencies. Strong communication skills, teamwork and a commitment to patient-centred care are essential. You will receive thorough induction, training and opportunities for ongoing development.
Responsibilities
- Work with other multi-disciplinary professionals within the team.
- Undertake initial assessments.
- Liaise with statutory and voluntary sectors.
- Contribute to meetings both within the service and in wider networks.
- Effectively plan and organise daily workload.
- Use flexible working and manage your own time according to service demands.
- Manage and provide a high quality of care.
- Manage a case load of patients with a range of mental health needs.
- Initiate, create and maintain therapeutic relationships with patients and carers, ensuring the practice is patient-centred.
- Participate in the duty rota.
- Practice in line with policy, guidance and legislation.
- Carry out risk assessment, crisis and safety planning.
- Work closely with the ACS mental health team to support patients’ mental health and Care Act needs.
- Support patients in being discharged back to primary care.
