Seeking a clinical pharmacist to join Lancashire Teaching Hospitals NHS Foundation Trust. The role involves rotations across specialties, patient care and medicines management within a team-based environment.
Main duties and responsibilities
- Provide a clinical pharmacy service to patients, wards and departments in accordance with clinical guidelines and Trust policies.
- Participate in medicines reconciliation on admission, clinical review and discharge processing as part of a team approach.
- Record individual clinical pharmacy involvement in patient care accurately.
- Provide professional and legal clinical supervision in the dispensary as allocated.
- Support safe medicines management arrangements including storage, incident reporting and risk minimisation.
- Act as a role model and supervise other pharmacy staff, including students and new pharmacists.
Qualifications & education
- MPharm or equivalent.
- Registration with the GPhC.
- Royal Pharmaceutical Society membership.
- Willingness to study towards a diploma in Clinical Pharmacy.
Knowledge & experience
- Fulfilment of mandatory continuing professional development requirements.
- Previous hospital experience.
