Primary Care Mental Health Practitioner

This role is an exciting opportunity for a registered band 6 mental health professional (Nurse, Social Worker or OT) to work across primary and secondary care to bring together mental health pathways and support the understanding of mental health within primary care. The role provides a holistic assessment of service users, links people into the community and VCSE resources, and facilitates seamless transition into secondary care when needed. It also forms part of multidisciplinary meetings held by GP practices within primary care.
The primary care mental health service is expanding and is a key component to our community mental health transformation. The role will closely link to the community transformation of mental health and the core team being established, and will work with the primary care network’s aligned practices. The successful candidate will be based in GP sites within the PCN during working hours.
Main duties of the job
- Actively promote better understanding and knowledge of mental health and wellbeing within primary care to enable the MDT to provide integrated and holistic offers of support.
- Take a lead on specialist assessment of service users with mental health problems within the primary care setting.
- Provide effective signposting based on a holistic assessment of an individual's needs, using a range of resources and services including VCSE and community organisations.
- Deliver evidence‑based, time‑limited interventions where appropriate and within scope of professional practice and experience.
- Facilitate swift and seamless transition into other mental health services such as CORE VCS, talking therapies, CMHT, etc., when required.
- Work collaboratively with secondary mental health services to support effective transitions of service users from secondary to primary care.
- Collaborate with service users and their families to support shared decision‑making, promote recovery and self‑care, and work on strengths and assets.
- Inform and influence the ongoing development of new and innovative models of support within primary care and secondary mental health service.

Person Specification
Experience
- Previous experience working with and holding a caseload of individuals with serious mental health problems.
- Previous experience working autonomously; assessing, developing care plans, and managing risk within a community setting.
- Previous experience working collaboratively within an MDT.
- Previous experience in service development, quality improvement, and bringing innovation and new ideas to practice.
- Previous experience coordinating care and signposting to relevant agencies and services.
- Previous experience managing responsibility or knowledge of Learning Disability services and ATU provision.
Qualifications
- Registered professional in Nursing (Mental Health), Social Work, or Occupational Therapy with current registration with NMC, Social Work England, or HCPC.
- Educated to degree level or equivalent experience.
