An excellent opportunity has become available for a highly motivated, flexible and passionate Advanced Clinical Practitioner to join the Older Adult Home Treatment teams. People currently undergoing ACP training are eligible to apply for band 7 and people who have completed the training are eligible to apply for band 8a.

Currently there are two Home Treatment Teams within the Older Adult Service that the post holder will work across:

  • Croydon Home Treatment Team based at the Bethlem Royal Hospital.
  • Lambeth, Southwark and Lewisham (LSL) Home Treatment Team based at the Maudsley Hospital.

The post holder will support the clinical team leader in providing clinical leadership and managerial oversight for the allocated caseload, and will support transfers across the Older Adult pathways, taking a lead in clinical decision‑making.

Main duties of the job

The post holder will deliver, and lead on the delivery of, high quality safe and contemporary care with the whole Older Adult Home Treatment service, ensuring consistently high standards of practice and clinical excellence, including: promoting population health, improving personal outcomes, contributing towards service evaluation, while leading and developing research and quality improvement initiatives.

Person Specification

Qualifications & Training

  • Post‑registration experience and/or evidence of formal courses and training at postgraduate diploma level.
  • Quality Improvement Methodology Training, e.g. LEAN.
  • Recognised Management or Leadership Qualification.

Experience

  • Extensive experience of leading the delivery of care for a diverse population.
  • Advanced level understanding of specialist‑specific disorders and their corresponding evidence‑based interventions.
  • Contemporary knowledge of relevant legislation and healthcare policies.
  • Applied understanding of issues relating to confidentiality and mental capacity.
  • Applied knowledge of safeguarding, inclusion and equality.
  • Experience working within healthcare services at an advanced level of practice.
  • Experience of writing for publication and/or presenting to professionals and the public.

Skills, Knowledge & Abilities

  • Advanced skills in undertaking clinical & risk assessments, specific to the area of practice.
  • Advanced level written and verbal communication skills.
  • Ability to manage own workload / work autonomously and determine / set own priorities.
  • Ability to build and maintain effective working relationships.
  • Effective self/time management skills to practice as an independent practitioner.
  • Advanced problem‑solving skills.
  • Skills in leading quality improvement initiatives.

Other

  • Acts as a role model in the delivery of high quality, effective patient care.
  • Able to cope under pressure and provide reassurance and direction to other staff members.
  • Be a reflective practitioner and develop own competence and support others to do so.
