Brookside Health Centre was established in the mid‑1980s and serves a population of around 11,000 people.
Main duties of the job
You will work alongside the GP and other colleagues providing full general practice level healthcare to our patients, with a proportionate caseload to support continuity.
In time you will take on clinical leadership for an area of interest or need within the practice.
Working hours are divided into sessions (morning and/or afternoon), including face‑to‑face and telephone appointments, the Hub telephone triage team, Duty GP cover and admin.
The team provides support via a buddy system for pathology and correspondence during annual leave, and all GPs provide clinical support and leadership to the practice.
While most home visits are undertaken by paramedics and a nurse consultant, you will be expected to conduct home visits as required during Duty GP sessions.

You will be part of a 7‑GP senior clinical team, working alongside 4 Advanced Nurse Practitioners, 3 Paramedics, a Nurse Consultant, Pharmacist, Mental Health Practitioner, Practice Nurses, General Practice Assistants, Social Prescriber, Dietitian, Care Coordinator, Health and Wellbeing Coach.

Job summary:
The post‑holder will manage a caseload and address a wide range of health needs in a primary care setting, maintaining high standards of care for all patients.
Main Clinical responsibilities
- Make surgery, telephone, and home‑visit consultations as per the practice timetable.
- Make autonomous professional decisions on presenting problems, whether self‑referral or referral from other health‑care workers.
- Assess health needs of patients with undifferentiated or undiagnosed problems.
- Screen patients for disease risk factors and early signs of illness.
- In consultation with patients and following disease‑management protocols, develop care plans.
- Provide counselling and health education.
- Admit or discharge patients to and from the caseload and refer to other providers as appropriate.
- Record clear consultation notes to agreed standards.
- Compile and issue computer‑generated acute and repeat prescriptions, avoiding hand‑written prescriptions when possible.
- Prescribe in line with the practice formulary or generically when clinically appropriate.
- Carry out all normal duties associated with a GP in primary care.
Other responsibilities
- Adhere to practice policies and guidelines, including prescribing, confidentiality, data protection, health and safety, safeguarding.
- Commit to life‑long learning and audit to ensure evidence‑based best practice.
- Contribute to evaluation, audit and clinical standard setting within the organisation.
- Contribute to the development of computer‑based patient records and coding.
- Attend training and events organised by the practice or other agencies.
Person Specification
Essential criteria include GMC registration, full CCT in General Practice or equivalent, and a commitment to continued professional development.
Qualifications
- Full GMC registration and inclusion on the Medical Performers List.
- Qualifications: CCT in General Practice, MRCGP, or Vocational Training Certificate.
- Focussed training and skills in a specialist area would be welcome.
Experience
- Excellent diagnostic, consultation and clinical management skills; able to work autonomously.
- Proficient in clinical IT systems (EMIS Web, SystmOne) and digital consultations.
- Excellent verbal, written, and active listening communication skills.
- Knowledge of NHS systems, QOF, and primary care challenges.
- Ability to work effectively in multidisciplinary teams.
- Commitment to personal and professional development and patient care.
- Resilient, adaptable, innovative, honest, and respectful attitude.
- Strong leadership and problem‑solving abilities.
- Interest in teaching, training or audit.
- Experience with minor operations and joint injections.
