District Nursing Lead - Advanced District Nurse Specialist

We are delighted to offer a career development opportunity within the District Nursing Service for District Nurse Specialists to progress to a senior, advanced clinical and leadership role.

This is a fixed term position to cover maternity leave for 1 year, based across the South East of Oxfordshire.

Applicants must have a full valid driving licence and access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.

Main duties of the job

You will be involved in developing and delivering the preventative care agenda, providing clinical leadership for local development of the national aging well anticipatory care model in Oxfordshire, and working closely with senior leaders. You will build relationships and integrated working with multi‑professional teams to deliver the model in local populations.

You will be responsible for ensuring that planned care within District Nursing teams in your area is in line with quality standards and best practice evidence‑based pathways. You will ensure that patients on the caseload have the best outcomes and that the caseload is effectively managed.

You will lead the DN teams in the management of patients who have more complex and/or advanced needs.

You will work in an integrated way to provide leadership and support for the locality DN teams with the District Nurse Lead and the locality's District Nursing Locality Lead post holders.
