Heart Failure Specialist Nurse

The post holder will work as a specialist practitioner within the Dudley Heart Failure Team both in the acute trust and community setting, assisting in the provision of a comprehensive service to an allocated caseload of patients with Acute or Chronic Heart Failure.

The service is currently operational minimum 0800-1600, 7 days a week (including bank holidays). The aim of the service is to facilitate a reduction in patient length of stay; a reduction of hospital admissions/readmissions and assist early discharge from hospital by expediting safe and timely pro-active support.

The post holder will be responsible for the delivery of comprehensive clinical assessment and care aimed at improving quality of life, supporting and empowering patients to manage their condition and improving the end of life experience for the patients and their carers.

The post holder will have responsibility for ensuring that his/her service reflects the KPI's of the Trust and will support the Heart Failure Lead and Matron in achieving these KPI's.
