Heart Failure Specialist Nurse

The post holder will work as a specialist practitioner within the Dudley Heart Failure Team both in the acute trust and community setting, assisting in the provision of a comprehensive service to an allocated caseload of patients with Acute or Chronic Heart Failure.

The service is currently operational minimum 0800-1600, 7 days a week (including bank holidays). The aim of the service is to facilitate a reduction in patient length of stay; a reduction of hospital admissions/readmissions and assist early discharge from hospital by expediting safe and timely pro-active support.

The post holder will be responsible for the delivery of comprehensive clinical assessment and care aimed at improving quality of life, supporting and empowering patients to manage their condition and improving the end of life experience for the patients and their carers.

The post holder will have responsibility for ensuring that his/her service reflects the KPI's of the Trust and will support the Heart Failure Lead and Matron in achieving these KPI's.

Main duties of the job

This may also include in-reaching into hospital in order to provide a seamless service.
• The service is provided 7 days including bank holidays. The aim is to facilitate a reduction in patient length of stay; a reduction of hospital admissions/readmissions and assist early discharge from hospital by expediting safe and timely pro-active support.
• The post holder will be responsible for the delivery of comprehensive clinical assessment and care aimed at improving quality of life, supporting and empowering patients to manage their condition and improving the end of life experience for the patients and their carers. .

Person Specification

Knowledge & Training

• A good working knowledge/understanding of NICE guidance for the management of chronic heart failure and a clear understanding of the role of the Heart Failure Specialist Nurse
• An understanding of primary care and community nursing
• A working knowledge of cardiological tests, particularly applicable to heart failure
• Cannulation and willing to work towards
• IV drug administration and willing to work towards

Planning Skills

• Vision and Enthusiasm - Proactive, Innovative, Committed to the principles of change
• Evidence of continued professional development
• Able to commute to meet the needs of the job

Education and qualifications

• Evidence of degree level study applicable to nursing
• Non-Medical Prescriber or willing to work towards
• Accredited Teaching and assessing in Clinical Practice qualification or equivalent
• Caledonian chronic heart failure qualification or other recognised accredited Heart Failure Qualification or willing to work towards.
• Accredited Qualification in Cardiology

Experience

• Experience of delivering patient education programs to patients with Heart Disease/Heart Failure
• Experience in palliative care
• Chronic Disease Management Experience / qualification
• Experience of managing Chronic Heart Failure

Analytical and judgement skills

• Confident in decision making and escalation
• Evidence of ability to analyse and interpret ECGs
• Ability to work within a team and independently
• Ability to establish effective working relationships and partnerships across both primary, secondary care and other health/social care organisations
• Presentation skills (with use of IT)
