Surrey and Borders Partnership NHS Foundation Trust is the leading provider of health and social care services for people of all ages with mental ill-health and learning disabilities in Surrey and North East Hampshire. We also provide social care services for people with a learning disability in Croydon and ASD and ADHD assessment services in Hampshire. Responsibilities
- Conduct comprehensive assessments of service users and create individualized care plans.
- Manage a caseload and deliver effective keyworker responsibilities.
- Establish and maintain therapeutic, collaborative relationships with service users, carers, and stakeholders.
- Communicate clearly and professionally with service users, families, colleagues, and external partners.
- Maintain accurate case notes and documentation in line with policy.
- Lead and contribute to service standards, audits, performance monitoring, and quality improvement initiatives.
- Undertake risk assessments, implement risk management plans, and respond appropriately to crises.
- Safeguard vulnerable adults and children in accordance with policies and procedures.
- Support and guide team members, acting as a role model for support workers and new staff.
- Engage in clinical supervision, ongoing training, and personal/professional development.
- Contribute to recruitment, induction, and performance management processes.
- Ensure compliance with health, safety, security, confidentiality, and organizational policies.
- Carry out detailed assessments of service users\' needs and develop tailored packages of care.
- Apply evidence-based practice and introduce innovative approaches when appropriate.
- Use clinical knowledge and skills to formulate, implement, and review care plans.
- Maintain therapeutic relationships and collaborative partnerships to ensure effective service delivery.
- Offer support, guidance, and leadership to support workers and new team members.
- Participate in staff recruitment, deployment, and management in line with SABP policy.
- Lead on developing, maintaining, and auditing service standards.
- Ensure care planning aligns with evidence-based practice, policy, and risk management.
- Maintain compliance with trust policies, standing orders, financial instructions, and confidentiality standards.
- Promote a healthy, safe, diverse, and inclusive working environment.
