Job Responsibilities
- Provide clinical assessment and management of patients on anticoagulation therapy (including initiation, monitoring, and dose adjustment).
- Prescribe and optimize anticoagulation treatment in line with local and national guidelines.
- Conduct patient consultations, review INR results, and ensure timely follow-up.
- Maintain accurate clinical records and contribute to audit and quality improvement initiatives.
- Collaborate with GPs, nurses, and other healthcare professionals to ensure seamless patient care.
- Support education and counselling for patients regarding anticoagulation therapy and adherence.
Qualifications
- Qualified Pharmacist with independent prescribing qualification.
- Current registration with the General Pharmaceutical Council (GPhC).
- Experience in anticoagulation management or relevant clinical areas.
- Strong communication and organisational skills.
- Ability to work autonomously and as part of a multidisciplinary team.
- Previous experience in primary care or community‑based anticoagulation services.
- Familiarity with clinical systems (e.g., EMIS, INR Star).
