Primary Care Network Advanced Care Practitioner

The post holder will be employed by Greenwich Health, the GP Federation in Greenwich. Greenwich has 7 primary care networks (PCNS) with list sizes of between 30-80,000 patients.The Post holder will work for Valentine Primary Care Network.

An ACP will demonstrate critical thinking and is a caring, compassionate and committed experiencednurse who, acting within their professional boundaries, will provide care forthe presenting patient from initial history taking, clinical assessment,diagnosis, treatment and evaluation of care. They will demonstrate safe,competent clinical decision-making and expert care, including assessment anddiagnostic skills, for patients within the general practice. They willcommunicate and work collaboratively with the general practice team to meet theneeds of patients, supporting the delivery of policy and procedures, andproviding nurse leadership as required.

The successful candidate must work a minimum of 2 days ( 4 sessions) and working face to face from up to 2 sites, depending on business needs.

Pro-rata participation in Extended Hours is essential for this role.

Main duties of the job

• Assess, diagnose, plan,implement and evaluate treatment/interventions and care for patientspresenting with an undifferentiated diagnosis
• Clinically examine and assesspatient needs from a physiological and psychological perspective, and planclinical care accordingly
• Assess, diagnosis, plan,implement and evaluate interventions/treatments for patients with complexneeds
• Proactively identify, diagnose,and manage treatment plans for patients at risk of developing a long-termcondition (as appropriate)
• Diagnose and manage both acuteand chronic conditions, integrating both drug- and non-drug-basedtreatment methods into a management plan
• Prescribe and review medicationfor therapeutic effectiveness, appropriate to patient needs and inaccordance with evidence-based practice and national and practiceprotocols, and within scope of practice
• Work with patients in order tosupport compliance with and adherence to prescribed treatments
• Provide information and adviceon prescribed or over-the-counter medication on medication regimens,side-effects, and interactions
• Prioritise health problems andintervene appropriately to assist the patient in complex, urgent oremergency situations, including initiation of effective emergency care
• Support patients to adopthealth promotion strategies that promote healthy lifestyles, and applyprinciples of self-care

Clinical Practice:

Communication:

• Utilise and demonstratesensitive communication styles, to ensure patients are fully informed andconsent to treatment

Delivering aquality service:

• Recognise and work within owncompetence and professional code of conduct as regulated by the NMC orother relevant professional body
• Produce accurate,contemporaneous, and complete records of patient consultation, consistentwith legislation, policies, and procedures
