We are looking for an enthusiastic Advanced Clinical Practitioner (ACP) to join our Medical Same Day Emergency Care (SDEC) team, managing over 100 patients daily across a wide range of acute medical conditions.

SDEC is an innovative, high-intensity service that provides patients with timely assessment, treatment, and discharge, without the need for traditional inpatient admission. As an ACP, you will work within a multidisciplinary team including consultants, doctors, physician associates, nurses, pharmacists, and other healthcare professionals, ensuring patients receive safe, effective, and personalised care.

This role offers an exciting opportunity to deliver acute care in a dynamic environment, using your clinical expertise to assess, diagnose, and manage patients efficiently. You will act as a clinical role model, support junior staff, contribute to team development, and demonstrate your skills across all four pillars of advanced practice. Dedicated time is provided for professional development, including opportunities for acute medicine hospital rotations.

The postholder will collaborate closely with the MDT while managing patient care independently, prioritising urgent needs, making complex clinical decisions, and escalating to senior colleagues as required. Flexibility to work weekdays, weekends, and bank holidays is essential, covering the full shift pattern.

Main duties of the job

The postholder will practice autonomously as an Advanced Clinical Practitioner within the Medical SDEC, with rotational experience in Acute Medicine services. They will provide patient‑centred care for a high‑volume acute medical caseload, delivering comprehensive assessment, examination, diagnosis, and treatment within an agreed scope of practice, and facilitating timely discharge or onward care as appropriate.

The postholder will be expected to maintain up-to-date professional knowledge, complete relevant training, and demonstrate ongoing development through the Trust‑wide portfolio.

Working both independently and as part of a multidisciplinary team (MDT), the postholder will be supported by the senior clinical team. They will be responsible for prioritising and triaging patient needs, making complex clinical decisions, and recognising when referral for further investigations or senior review is required, in line with local and national guidelines.
