Main duties of the job
As Team Leader, you will oversee the smooth running of sessions, guide and support a team of Support Workers, and ensure high quality, person centred care. You will also build local relationships, raise awareness of the service, and contribute to support long term sustainability.
This role is ideal for someone with strong organisational skills, a positive approach, and a passion for supporting people living with dementia.

Summary of post-
The post holder will provide effective day to day support to a team ofSupport Workers, ensuring the safe, person centred, and high quality deliveryof activities for people living with dementia. They will oversee thecoordination of the service, maintain high operational standards, and supportstaff performance and wellbeing. They will also develop and maintain localrelationships, promote the service, and increase the uptake of privately fundedclients to support long term sustainability and growth.
Key activities and mainresponsibilities-
- To oversee the smoothrunning of Dementia Day Opportunities sessions, ensuring activities arewell planned, aligned to the services core principles meaningful, andresponsive to the needs and abilities of attendees.
- To provide clearoperational guidance to staff, ensuring consistent delivery of high quality,person centred support and ensuring appropriate staffing and cover across corehours.
- To ensure all client assessments,reviews, and related documentation are completed accurately and in a timelymanner.
- To providesupervision to support workers, including regular check ins, clearcommunication, timely follow up, and annual appraisals.
- To support staffdevelopment through coaching, training, and modelling best practice indementia inclusive approaches ensuring workforce capacity meets service demand.
- To proactivelymonitor performance, including attendance levels, staffing and capacity, takingearly action to address shortfalls.
- To oversee theaccurate and consistent use of internal databases and systems, data entry andusing dashboards to optimise service delivery and reporting.
- To deal promptly andappropriately with issues relating to attendees, including safeguardingconcerns and complaints.
- To actively promotethe Dementia Day Opportunities service through building strong relationshipswith local partners, community groups, health professionals, and referralpathways.
- To identify opportunitiesto increase the uptake of privately funded clients
- To raise awareness ofthe service through outreach, events, and local engagement.
- To portray a positive image of theorganisation both internally and externally, and set high standards of personalintegrity and professionalism, leading by example.
- To ensure all staff adhere to AUKCWpolicies, procedures, and quality standards at all times.
- To undertake any other duties as may bereasonably required, consistent with the nature and grade of the post.

Person Specification
Experience
- Experience of working in health, social care or related support services.
- Ability to motivate, guide and support staff to deliver high-quality services and meet performance targets.
- Strong planning and prioritisation skills, with the ability to work proactively and independently while coordinating team activity.
- Excellent verbal and written communication skills.
- High digital competence, including Microsoft Office, email, databases, and the ability to learn systems with strong skills in collecting and interpreting electronic data for monitoring and reporting.
- Ability to represent the service professionally and build positive relationships that support referrals and growth.
- Professional, compassionate, and committed to improving the lives of people with dementia, while remaining approachable, supportive, and able to build trust with families and the team.
- Experience of supporting people living with dementia or older adults with complex needs.
- Line management experience, including supervision, performance monitoring and supporting staff development.
- Experience of leading or coordinating a team within a care, support or community-based setting.
- Knowledge of dementia-inclusive practice and person-centred approaches.
- Experience of contributing to service promotion, outreach or income-generating activity.
Qualifications
- Relevant qualifications in health, social care or related field.
