Position Overview

An excellent opportunity has become available for a highly motivated, flexible and passionate Advanced Clinical Practitioner to join the Older Adult Home Treatment teams. People currently undergoing ACP training are eligible to apply for band 7 and those who have completed the training are eligible to apply for band 8a.

There are two Home Treatment Teams within the Older Adult Service with which the post holder will work across:

• Croydon Home Treatment Team based at the Bethlem Royal Hospital
• Lambeth, Southwark and Lewisham (LSL) Home Treatment Team based at the Maudsley Hospital

Key Responsibilities

• Support the clinical team leader in providing clinical leadership and managerial oversight for the allocated caseload.
• Take a lead in clinical decision making and support transfers across the Older Adult pathways.
• Deliver and lead on the provision of high quality, safe and contemporary care with the whole Older Adult Home Treatment service, ensuring consistently high standards of practice and clinical excellence.
• Promote population health, improve personal outcomes, contribute towards service evaluation, and lead and develop research and quality improvement initiatives.
