Salary: £39,959 – £48,117 gross per annum (pro rata). Salary period: Yearly.
Job overview
We are excited to announce a Band 6 Senior Community Mental Health Practitioner post within our Adult Community Mental Health Team covering Great Yarmouth and surrounding area.
As a Band 5/6 practitioner with at least 24 months post‑registration experience, you will take responsibility for planning and delivering direct patient care with minimal supervision, ensuring continuity of practice in a multidisciplinary team/multi‑agency environment.
Main duties
- Manage a caseload and provide mental health care services to people with mental health needs.
- Conduct planned care, risk assessment, screening and ongoing evaluation of patients’ needs; identify suitability for potential discharge in conjunction with the Senior Nurse.
- Initiate and attend patient‑centred reviews, case conferences and other meetings as required.
- Advise the Responsible Medical Officer(s) and/or health and social care professionals of any change in patients’ conditions and circumstances.
- Work within the multidisciplinary team and form partnerships with other care providers.
- Enable and empower service users/carers to make informed decisions about their care.
- Provide caseload management supervision to junior staff under the direction of the Senior Nurse; assist with the supervision of peers if requested.
- Help patients gain and maintain independence.
- Identify personal learning and training needs through KSF, appraisal and personal development plans on an annual basis.
- Supervise student placements within the team.
Person specification
Experience
- Experience within a multi‑disciplinary team.
- Experience delivering person‑centred treatment packages.
- Experience in mental health settings.
- Community experience including care coordination.
Qualifications
- NMC Registered Nurse, Social Worker, Occupational Therapist or other relevant professional qualification.
- Willingness to train to become Sign‑off Mentor.
Knowledge
- Knowledge of medication and monitoring of its effects.
- Knowledge of psychosocial interventions.
- Developing knowledge of local wider systems and networks.

- Ability to travel independently.
