Community Mental Health Practitioner

We are pleased to share this opportunity for a motivated and enthusiastic mental health professional to join the Birkenhead Community Mental Health Team. (Qualified mental health Nurse).
The successful candidate will join an established, knowledgeable and enthusiastic team that supports each other, delivering an exceptional service for its patients. Daily duties include clinic work, assessment, formulation, care planning, risk assessments, reviews and tailored interventions, plus offering signposting to other agencies where necessary.
Responsibilities
- Work within a multidisciplinary framework, undertaking assessments and coordinating care of patients with functional and organic mental illnesses.
- Support or lead aspects of busy community clinics.
- Manage and assess a manageable caseload, including ongoing risk assessment, care planning and monitoring of outcomes.
- Provide tailored interventions to improve patient wellbeing and outcomes.
- Maintain excellent communication, report writing and assessment skills.
- Provide professional support to patients via the whole team approach.
Qualifications and Experience
- Appropriate professional qualification.
- Evidence of continuing professional development.
- Minimum of 2 years post-qualified experience in a community mental health setting at band 5 or 6.
- Experience of implementing significant changes in working practices, monitoring standards of care, providing supervision, and working within a multidisciplinary team.
- Working knowledge of memory and functional disorders and relevant legislation.
- Evidence of undertaking audit work.
