Role
We are seeking a Registered Mental Health Nurse, Occupational Therapist or Social Worker to work within the multidisciplinary team with Mental Health Together. There are two part‑time roles available.
Scope of Practice
As a Community Mental Health Practitioner, you will assess, plan, coordinate, deliver and evaluate care for adults (18 yrs+ ) in the South West Kent area who are experiencing mental health disorders. You will work in a community setting, using evidence‑based, client‑centred, hope and recovery, health promotion, relapse prevention and socially inclusive principles.
Key Responsibilities
- Carry out comprehensive, systematic mental health assessments, considering mental, physical, social, cultural, psychological, spiritual and environmental factors, in partnership with service users and others.
- Collaborate with other disciplines, service users and carers to agree a holistic, person‑centred care plan addressing identified needs.
- Deliver and evaluate safe, person‑centred care in partnership with service users and their carers to support recovery.
- Provide educational support to optimise health and wellbeing; promote self‑care where possible, help people make choices about their needs and involve carers where appropriate.
Required Qualifications & Experience
Applicants must be registered as a Mental Health Nurse, Occupational Therapist or Social Worker and have experience working with adults in community mental health settings.
