This is a new and exciting post created within the Bolton Specialist CMHTs to enhance the support offered to service users with an SMI and a co‑occurring condition.

Responsibilities

  • Lead a cluster team within the CMHT in line with the Community Transformation model offer.
  • Provide assessment, formulation, brief and extended interventions, and collaborate on care planning with the MDT and wider substance misuse services.
  • Strengthen pathways into substance misuse support and improve continuity of care between inpatient and community teams, and between specialist CMHT and external providers.
  • Lead the co‑occurring conditions cluster within the community transformation model.
  • Deliver and embed training for CMHT keyworkers.
  • Hold a small caseload or work jointly with other teams.
  • Contribute to Rapid Reviews of Care and facilitate reflective supervision groups.
  • Ensure people are not excluded from mental health or substance misuse services due to co‑occurring conditions.
  • Establish and maintain strong multidisciplinary relationships.
  • Cover the Bolton borough and travel as required.
  • Join the operational on‑site/on‑call rota and deputise for team managers as required.
