Overview
This is an exciting opportunity to take a lead role in contributing to the delivery of high-quality care within the South Manchester Home-Based Treatment Team. As a Band 7 Senior Practitioner, you will provide strong leadership and support the management of a busy urgent care team to ensure the delivery of safe, effective, and compassionate care for individuals requiring HBTT as an alternative to hospital admission. The Senior Practitioner will support the day-to-day coordination and management of the service, provide visible and accessible leadership to staff within the Service, support the delivery of a safe and efficient service to those accessing it, and ensure timely, person-centred interventions that promote recovery and wellbeing. Key aspects include monitoring the performance and quality of the service the team delivers alongside clinical leadership and effective day-to-day management of the team.
The HBTT service operates 24 hours a day, 7 days a week, so flexibility in working patterns is essential, which may include weekend and out-of-hours working and contributing to the on-call Operational rota.
Responsibilities
- Provide visible and accessible leadership to staff within the Service; support the delivery of a safe and efficient service to those accessing it and ensure timely, person-centred interventions that promote recovery and wellbeing.
- Monitor the performance and quality of the service the team delivers; alongside clinical leadership and effective day-to-day management of the team.
- Manage your own workload, work well under pressure, and work within a multi-disciplinary team; collaborate with multiple agencies; demonstrate excellent interpersonal skills and an awareness of Trust Policies and Procedures.
- Support the service to develop and maintain robust gatekeeping processes, incorporating the principles of person-centred care and least restrictive interventions.
- Proactively identify and facilitate supported early discharges from inpatient settings by attending board rounds and mini-MDTs, and completing trusted assessment for patients going to out-of-area HBTs on discharge.
- Be involved in purposeful admission formulation and statements, and the review of these as part of ward rounds if admitted and with other services involved such as community mental health teams.
- Contribute to leadership and service development of the HBT, including line managing staff, offering clinical supervision (formal or informal), attending leadership meetings, and supporting operational duties.
- Facilitate spoke days for shadowing as part of induction or student placements (from HBT or other services across the division).
- Support the provider in leadership and service development of the HBT and related activities.
- Plan with the multidisciplinary team the allocation of key workers and involvement of other team members in treatment plans.
- Undertake risk assessment and risk management of all clients as necessary.
Requirements
- Band 7 Senior Practitioner with the ability to lead and manage a busy urgent care team in the HBTT.
- Flexibility to work a 24/7 service pattern, including weekends and out-of-hours on-call rota.
- Excellent interpersonal skills and the ability to work collaboratively with multiple agencies; awareness of Trust Policies and Procedures.
- Experience in line management or supervision; ability to provide clinical supervision.
- Experience with gatekeeping, least restrictive interventions, and person-centred care.
