Advanced Clinical Practitioner (ACP)

Are you a highly motivated, experienced Advanced Clinical Practitioner, looking for a new challenge? This role is based in the Mansfield LMHT and may develop over time in response to changes in service delivery.
Main Duties of the Job
- Demonstrate safe, senior clinical decision-making by autonomously assessing patients, including initial history, clinical assessment, diagnosis, treatment, and evaluation of care.
- Demonstrate critical thinking in the clinical decision-making process and work collaboratively with the MDT.
- Guide junior medical staff and nursing staff in relation to relevant protocols, procedures, and clinical pathways.
- Be fundamental in the development of this new role.
- Initiate, manage and drive innovative change in practice for the benefit of patient care in line with Trust and local procedures.
- Provide mentorship to trainee ACPs within the directorate.
- Serve 1st on call duties on a 3‑month rota basis.
- Act as a clinical leader for a fully qualified ACP at Masters level.
- Possess a full UK driving licence and vehicle for business use.
Job Responsibilities
- Complete a full system and physical or psychological/psychiatric exam.
- Take a systemic and/or psychosocial patient history for service users or prisoners.
- Make diagnostic decisions based on consultation, history, presentation, pathology, and/or imaging results.
- Initiate, prescribe and monitor the impact and outcome of treatments and medication.
- Devise an individual clinical management plan including all aspects of multi‑disciplinary care.
- Screen patients for early signs of disease and risk factors.
- Carry out appropriate invasive and non‑invasive diagnostic and therapeutic procedures.
- Refer to other health professionals as appropriate.
- Discharge from hospital or services as required.
- Run independent out‑patient clinics seeing new and review individuals as required.
- Accountable for independent clinical assessment, formulation, diagnosis and treatment of undiagnosed conditions.
- Practice in compliance with professional conduct and scope of practice.
- Demonstrate critical understanding of broadened level of responsibility and autonomy.
- Act in professional judgement when seeking help, reflecting on practice.
- Work in partnership with individuals, families and carers, using a range of assessment methods.
- Demonstrate effective communication skills supporting people in decision-making.
- Use expertise to inform clinical reasoning with evidence‑based judgments.
- Initiate, evaluate and modify a range of interventions including prescribing medicines and therapies.
- Exercise professional judgement to manage risk and support teams.
- Work collaboratively with multi‑agency and inter‑professional resources.
- Act as a clinical role model and advocate.
- Demonstrate underpinning subject‑specific competencies.
Qualifications
- Current registration with the NMC or HCPC.
- Clinical based Post‑graduate Degree.
- MSc Advanced Practice.
- Management/Leadership Qualification or equivalent.
- Recognised teaching or mentorship qualification at degree level or equivalent.
- Non‑Medical Prescriber (V300).
Training
- Evidence of continued professional development.
Experience
- Evidence of working at a senior position within Health Care of Older People or equivalent services.
- Demonstrate ability to work across all 4 pillars of Advanced Clinical Practice.
- Experience working with older people with mental health issues and complex physical comorbidities.
- Evidence of extensive clinical knowledge managing unwell patients in the community, primary or acute settings.
- Specialist clinical training and CPD in patient history taking, clinical examination and developing management plans.
- Knowledge of national and local NHS healthcare agenda, standards, research and its implications.
- Participation in research, audit and evaluation.
- Knowledge and experience of chronic disease management and long‑term conditions.
- Evidence of managing staff and multidisciplinary teams.
- Evidence of relevant involvement in meeting the Trust clinical governance objectives.
- Understanding of the Mental Health Act 1983, Mental Capacity Act 2005 and DOLS.
- Demonstrate assertiveness, tact and diplomacy.
- Evidence of involvement in policy and practice change.
- Evidence of proactive involvement in mentorship and development of nurses.
- Computer literacy.

Salary
£57,528 to £64,750 a year (pro‑rata for part time).
Additional Information
Applicants will be required to pay for their DBS check. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced) and will be deducted from salary over the first 2 months of employment.

Enrolled for DBS Update Service, annual fee of £16 per year applies.
