Staff Nurse

Location: Wyre Forest Ward, Kidderminster Hospital.
We are looking for a Staff Nurse to provide high quality rehabilitation care to patients admitted to our ward. This part‑time role is currently 347.5 hours per week but we will consider fewer hours for the successful candidate.
Main duties of the job
As a Staff Nurse you will be responsible for providing care to people admitted to our ward for rehabilitation. We work closely as a multi‑disciplinary team. Our focus is promoting the comfort and wellbeing of patients, enabling them to live as independently as possible. Our nurses facilitate the day‑to‑day running of the ward, administer medications, and work alongside all members of the MDT, patients, families and carers to provide 24/7 rehabilitation to enable the highest level of independence and quality of life. Nurses attend meetings such as MDT, CHC or discharge planning as required.

Job responsibilities
The post holder will:

- Be responsible for the assessment, planning, implementation and evaluation of patient care within the ward, working in accordance with the philosophy of patient‑centred care and to NSF and other Trust standards.
- Practice as a member of the Community Hospital Nursing team, to support and work in partnership with the wider multidisciplinary team.
Planning and Delivery of Individual Nursing Care under Direction of Senior Staff
- Work within the NMC Code of Professional Conduct.
- Assess, plan, implement and evaluate patient‑focused nursing care.
- Document care given and maintain contemporaneous records.
- Maintain records in accordance with the Trust’s policy and the NMC Code of Professional Conduct.
- Assist in the implementation of health promotion strategies, as agreed by the Ward Manager.
- In consultation with senior qualified staff, provide and monitor patient‑centred care within Trust agreed standards.
- In collaboration with other team members organise the reception and discharge of patients ensuring effective communication in the transfer of information.
- Carry out delegated nursing care.
- Use and participate in research or audit in care delivery.
- Adhere to and promote all clinical practice, health and safety and corporate Trust policies and guidelines.

Experience
- Experience gained during pre‑registration training
- Experience working in a frailty care setting
Qualifications
- Registered with RGN/RMN
- Registered with the NMC
- Original certificates to verify qualifications will be required at the point of offer
- Practice Supervisor and/or Practice Assessor training
- Accredited courses in Palliative Care, Tissue Viability, Diabetes, Dementia
Additional Criteria
- Responsible for own professional development
Knowledge
- Confidence to teach student nurses, patients, carers and members of the multidisciplinary team
- Understanding of evidence‑based nursing, multidisciplinary working, clinical governance
- Awareness of audit
- Evidence of ongoing continuing professional developmentExperience of audit
- Knowledge and understanding of Mental Capacity and DOLs
