Overview

Our highly skilled, forward‑thinking Urgent Care Service in West Kent brings together nurses, therapists, frailty specialists, GPs, consultants and advanced practitioners to deliver urgent, specialist and proactive care that helps people living with frailty remain safely at home. With the Frailty Virtual Ward (Home Treatment Service) and proactive multidisciplinary pathways delivered through the Integrated Care Management MDT, we are seeking an additional Consultant ACP to work alongside our existing consultant practitioners and ensure senior clinical oversight seven days a week.

Key Responsibilities

  • Lead and oversee the Frailty Virtual Ward and UCR caseload, providing senior clinical oversight seven days a week.
  • Autonomously assess, diagnose and manage highly complex frailty presentations across community, virtual ward and urgent care pathways.
  • Lead all four pillars of Consultant‑level practice: expert clinical practice, strategic leadership, education and research.
  • Provide values‑based leadership across services and systems, support staff to develop, innovate and deliver excellence, mentor ACPs and trainee ACPs, and provide clinical supervision.
  • Drive research, evaluation and service transformation, fostering a knowledge‑rich, inquiry‑driven culture and contributing to national best practice.

Qualifications & Experience

  • Recognised clinical expert, preferably with advanced practitioner experience in frailty care.
  • Strong clinical decision‑making skills with the ability to work autonomously across community, virtual ward and urgent care pathways.
  • Demonstrated leadership in multidisciplinary settings, including mentorship and clinical supervision of ACPs and trainees.
  • Commitment to values‑based, person‑centred care and evidence‑based practice.
