This vacancyis for an experienced, caring Pharmacist to join our established team inSherborne.

As a Pharmacistyou will provide expert clinical advice on medicines and polypharmacy. Ensuringthe provision of safe, modern, medicines and personalised care. Taking aproactive role in medicines optimisation, in line with contractual obligationsand expected standards. Support the repeat prescription service and undertakemedication reviews. Deliver an integrated, safe, cost effective andhigh-quality pharmacy service, as part of a comprehensive healthcare packagebased on our population needs.

Inspirepositive and aspirational leadership to the team of pharmacy technicians andthe wider healthcare team in order to maintain a good working environment.

Main duties of the job

Work closelywith the GPs and other healthcare staff to promote good practice in allprescribing decisions, to develop and implement improved medicines management.Helping to reduce waste whilst maintaining the focus on patient safety andwell-being.

Shareinformation relating to pharmaceutical products, medicines management,medicines optimisation, medicines legislation and general health education sothat patients and colleagues are appropriately informed. You would be expectedto be responsible for an area of chronic disease management.

Support thegeneral practice staff with prescription and medication queries and help toenable partnership working with community pharmacies and secondary care.

The postholder will help to recognise and improve processes, supporting the team to continueto deliver excellent healthcare. Analysing data to produce reports andrecommendations to support population health management, helping the team tocomplete projects through to a successful and timely conclusion.

About us

You willneed to be flexible and used to working independently, whilst forming part of awider team. Sherborne has a team of three Prescribing Pharmacists, and three Pharmacy Technicians who work the majority of their week at one of our three practices, serving a total patient list of approx. 25,000. You will be contributing directly to our populations ongoinghealth needs in this vital patient facing role.

Employed bySherborne Primary Care Network, the successful applicant will work primarily atone practice, to support the delivery of care to all our patients.

You will possess the ability to delivercomplex and sensitive information appropriately to patients, and delivereducational sessions/information to other healthcare professionals. You will be adaptable, with excellentcommunication skills and a proven ability to work under pressure and meetdeadlines.

The successful candidate will be able to work on their own initiative, organisingworkload, and work well as part of a team.

This post is for 30 hours per week, although there may be some scope for the right candidate
