Our new neighbourhood based services are centred around GP Primary Care Networks and arranged to increase engagement with Integrated Care Systems in the Hammersmith and Fulham Area.

Utilising a whole population health approach, the model uses all community resources, including health, social care, voluntary community and social enterprise organisations and local communities to support wellbeing beyond mental health services alone.

Mental Health Practitioner

As a Mental Health Practitioner you will be employed by West London NHS Trust and will be key in developing pathways of care in the local community mental health team (MINT). The role offers opportunities for career progression and continued education.

Ideal Candidate

• Registered Mental Health Nurses, Social Workers or OTs with community experience
• Prior experience across a range of community mental health settings and with diverse mental health diagnoses
• Knowledge of delivering brief interventions to patients with mental health needs
• Good written and verbal communication skills to strengthen local relationships with MINT teams
