Thirteen Group is on the lookout for a Support Service Co-ordinator on a 6-month secondment. You’ll be based at our young parent supported housing scheme at School View in Durham to support young families that need support to be able to live more independently.

Key Responsibilities:
- Run activities linked to seasonal events or celebrations or activities that help improve social isolation and specific learning linked to support. We’ll give you the creative freedom to provide positive outcomes for our customers.
- Support with any referrals received and assessing suitability for the service.
- Work with individuals to create person‑centred support plans and hold weekly keywork sessions to achieve their goals identified.
- Manage risk and safeguarding for all residents.
- Attend multi agency meetings and provide written reports.
- Manage void properties and allocations.
- Responsible for managing building compliance and health & safety of the scheme.
- Assist individuals to maximise their income and apply for relevant welfare benefits and grants.
- Working as a team of four, you’ll work across a two week rota, covering our operating hours of 9am to 5pm on weekdays. You’ll be required to work one in two Saturdays, and the weekend you work, you will have the Monday after off. You'll be expected to be on call one in every four weeks.
- You’ll also have the opportunity to pick up additional hours on a relief basis, working across other schemes to gain insight into other parts of the business and create strong working relationships across teams.
About You:
- You’ll have experience of supporting vulnerable client groups, along with a good understanding of adult and child safeguarding and the complexities that it might bring.
- This role involves lone working at times, so you’ll be able to thrive in your own environment, as well as part of a team.
- The role also includes an element of supporting complex families, in their own home, on an outreach basis across County Durham and therefore, a driving licence and access to your own vehicle is essential.
- Able to prioritise your workload and supportive in a non-judgemental way as you’ll be responsible for enabling our customers to reach their potential and for identifying their goals through individual support plans.
- You’ll support our customers in a flexible and person-centred way, so you must have emotional resilience and the ability to deal constructively with challenging behaviour.
