General Practitioner with an Extended Role (GPwER) – Initial Health Assessments for Looked‑after Children
Location: Manchester, Trafford (Community Children’s Services – Manchester)
About the role
You will provide one weekly Initial Health Assessment (IHA) for Looked‑after Children (LAC). Operating within a patient‑centred ethos, you will empower children to take control and self‑manage their health, undertake the initial assessment, and determine and enable any further support needed.
Key responsibilities
- Conduct the weekly Initial Health Assessment for Looked‑after Children.
- Collaborate with a wider team of clinicians and operational colleagues.
- Use excellent communication skills to support operational delivery of initial health assessments in Manchester and Trafford.
- Determine and enable further support required following the assessment.
Qualifications and experience
- Established General Practitioner with knowledge of local community services.
- Strong communication skills and the ability to work collaboratively in a multidisciplinary team.
- Experience in delivering patient‑centred care and in coordinating support for children and young people with complex needs.
