Role Overview
The post holder will champion a frailty approach to care and use comprehensive geriatric assessment to achieve the best possible outcomes for older adults living with frailty in the Trust. They will be responsible for the care of 12‑14 patients on a 25‑bedded, acute delirium and dementia ward, providing clinical supervision to trainee doctors and leading the ward‑based MDT.
Clinical Responsibilities
- Provide clinical supervision to the Acute Frailty Team and advanced clinical practitioners.
- Work with the teamwork in ED and on the MAU to complete acute comprehensive geriatric assessments for older adults presenting with frailty crises, and provide senior‑level reviews.
- Develop a weekly clinic tailored to the successful candidate’s interests.
- Participate in the unselected acute medical take rota.
- Work flexibly to achieve high‑quality care, with particular emphasis on:
- implementing and achieving the targets set by NHS England for older people and frailty care and helping achieve the targets set by GIRFT;
- ensuring NHS Standards for out‑patient waiting times are achieved;
- ensuring optimal use of all resources allocated to the post;
- developing appropriate services and techniques required to meet clinical needs within available resources;
- working together with colleagues to ensure effective management of inpatients, internal referrals, outpatient clinics and investigational sessions;
- working together with primary‑care colleagues to ensure effective management of community patients;
- participating in the education and training of medical students, doctors, and allied‑health professionals, including those in primary care;
- demonstrating a firm commitment to the principles of clinical governance;
- developing and maintaining robust systems for effective clinical audit in the speciality and general medicine, as appropriate;
- participating in developing services as indicated by recommendations of the Royal College of Physicians, NICE and specialist societies about development of a peri‑operative service unit.
Compensation
NHS Medical and Dental salaries are set at a national level. For new starters not already working to UK Medical and Dental terms and conditions of service, the paypoint will be set to the minimum level and can be increased only with evidence of relevant, completed year’s experience in terms of nature, type and level of role. Evidence required to increase a starting salary will be payslips and details noted in the application form, i.e. complete employment history.
