Band 6 Duty and Care Co-ordinator Nurse – Longbridge CMHT

Longbridge CMHT is looking for a Duty worker to support the team in managing the busy CMHT duty system for part of the role, and managing a small caseload for the other part. The successful individual will work with a dedicated and hardworking team to work with individuals who are in crisis or in need of urgent intervention, alongside working across CMHT in supporting caseload needs.
Main duties of the job
The post holder will act as an independent practitioner within a defined community setting, while liaising with other professionals as necessary.
- Responsible for assessment, planning, implementation and evaluation of nursing care within agreed parameters.
- Maintain working knowledge of local, national policies and legislation governing current service provision and audit.
- Ensure effective communication pathways exist.
- Promote equality, diversity and rights in accordance with good practice and legislation.
- Identify and contribute to the training needs necessary in providing quality evidence‑based care as appropriate.
