Care Co-ordinator Maternity Cover (1 year) Job Description: At Bluebird Care Coleraine we believe that the secret to outstanding care provision is to value, support, and engage with our team. We are seeking a Care Co-ordinator to join our Coleraine based team. We are looking for an exceptional individual who has the ability to remain calm under pressure and is able to build and utilise excellent interpersonal skills with our Customers, Staff and Health Professionals. They should be self-motivated, and able to work to tight deadlines when required. The Care Co-ordinator is a pivotal role within the organisation, ensuring that all Customers needs are met enabling them to safely and independently remain in their own homes. The Co-ordinator is also responsible for ensuring that our Care Assistant employment needs are also taken into consideration. The role will also include provision of an out of hours/on-call service based on a rota system. High level of literacy, numeracy, and confident knowledge of computer systems is essential. Experience of PASS System would be advantageous but not essential as full training will be provided. Previous experience within the health and social care sector is desirable. Key Elements of the Care Co-ordinator role Overseeing the provision of care to Customers Liaise with Registered Manager to ensure sufficient recruitment of Staff to cover calls Allocate bookings to Care Assistant roster, ensuring a good work/life balance is met. Ensure there is sufficient cover to deliver care and support to all customers on time and in alignment with their personal care plans. Make adjustment to rotas as required to provide cover for emergency situations, to cover staff holiday or sickness, etc Work with the office team to maintain up to date electronic and handwritten records. Ensure that concerns, issues, accidents and incidents are recorded, reported and acted upon. Liaise with customers and their chosen representatives about their care and support, taking account of different communication needs and levels of understanding. Work with the Bluebird Care team and other health and social care professionals to deliver high quality homecare services and to make improvements where necessary. Carry out general office duties and prepare reports as required by the Registered Manager. Work flexibly as part of the team, and apply Bluebird Care policies and procedures at all times.
