Consultant Advanced Clinical Practitioner (ACP) – Frailty

Eligibility: Open to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies also open to Student Nurses and Newly Qualified Nurses.
About the Role
The Consultant ACP for Frailty will join a highly skilled Urgent Care Service in West Kent. The service delivers urgent, specialist and proactive care that helps people living with frailty remain safely at home, through the Frailty Virtual Ward and proactive multidisciplinary pathways.
Key Responsibilities
- Lead the service as a senior clinical decision-maker across all four pillars of Consultant-level practice: expert clinical practice, strategic leadership, education and research.
- Autonomously assess, diagnose and manage highly complex frailty presentations across community, virtual ward and urgent care pathways.
- Provide values‑based leadership across services and systems, supporting staff to develop, innovate and deliver excellence.
- Mentor ACPs and trainee ACPs, guide them in developing their four pillars of practice and provide clinical supervision.
- Foster a knowledge‑rich, inquiry‑driven culture that supports research, evaluation and service transformation.
- Use consultancy approaches to influence practice across organisational boundaries and contribute to national best practice.
- Drive to perform duties where required, holding a full UK or EU driving licence.
Qualifications
- Experience at band 8a or above.
- First level Registration with NMC, HPCP or GPHC.
- Teaching qualification.
- Research qualification.
- Leadership skills.
- Non‑medical prescribing.
- Full UK/EU driving licence.
- Educated to Master’s level or higher degree.
- Psychology or counselling experience and training/education.
Experience
- Current Independent prescriber.
- Extensive clinical experience in the care and treatment of people with frailty (including long‑term conditions and dementia).
- Advanced skills in history taking and physical assessment of frail people.
- Experience of advanced specialist interventions in assessment and management of older patients.
- Experience of teaching and education provision.
- Extensive experience in report writing.
- Experience of planning, implementation and delivery of educational programmes relating to long‑term conditions.
- Experience of working across organisational boundaries with partners (commissioning, social care, education providers).
- Advanced IT skills.
- Experience with evidence of audit, research and innovation applied in practice.
- Experience of strategy development and implementation.
- IR(ME)R training or equivalent.
- Extensive experience of presenting report/education/information to public forums.
Knowledge
- Advanced understanding of long‑term conditions and application to older people.
- Advanced knowledge of clinical governance.

Salary
£66,582 to £77,368 a year pro‑rate per annum.
