Type: Permanent | Band 7, 36hrs/ 1 WTEt
Overview
The Hospital at Home service within NHS Borders is expanding. This post will provide secondary level care within the patient’s own home or care setting. The ANP will be responsible for delivering safe, effective, high quality care and ensuring continuity of care outside a hospital environment.
Key Responsibilities
- Provide clinical care to patients who would otherwise be in a hospital setting.
- Work collaboratively with acute and community teams to deliver secondary level care.
- Make independent clinical decisions and supervise registered and unregistered staff.
- Liaise daily with GP practices, local authority teams and the voluntary sector.
- Work flexible hours, including weekdays and weekends on a rotational basis; current service runs 7 days, 08:00-18:00.
Qualifications
- Full UK NMC registration.
- Full UK driving licence.
- Flexibility to work weekdays and weekends on a rotational basis.
