Position Overview
Provide integral support to the operational duties of the Integrated Discharge Hub that operates seven days a week, 8am‑8pm. The Discharge Officer is ward‑based and supports the MDT by collating key information and sharing it with appropriate professionals/services to ensure safe discharge.
Key Responsibilities
- Work on a rota basis 7 days per week, covering shifts between 08:00‑20:00 (7.5‑hour shifts).
- Primarily support one ward with flexibility to cover others and support operational activity.
- Facilitate effective discharge planning, escalating delays and ensuring timely, accurate, patient‑centred communication.
- Collaborate with the Discharge Practitioner, Operations Centre, ward teams, patients, families and external multi‑disciplinary partners.

- Excellent communication and interpersonal skills.
- Strong problem‑solving ability.
- Capacity to work independently and collaboratively.
- Confidence working in a busy and demanding environment.
- A positive, patient‑centred approach at all times.
