Improvement Facilitator

Bradford District Care NHS Foundation Trust is looking for a passionate and motivated individual to support our Improvement and Innovation Team, helping to deliver consistent, sustainable improvements across Mental Health services.

This role focuses on implementing the Continuous Improvement System, improving patient flow across community pathways and inpatient wards, and embedding a Culture of Care that promotes compassion, dignity and therapeutic relationships.

Main responsibilities

  • Work collaboratively with Mental Health inpatient and community teams to identify issues affecting flow, including delays, bottlenecks and inconsistent working practices.
  • Support teams to improve flow through activities such as reducing delays at triage, assessment or admission, strengthening discharge planning and community follow‑up, improving multidisciplinary team coordination and communication, and reducing avoidable length of stay and variation across wards.
  • Help teams test, implement and embed new standards of work that support smoother, safer transitions between services.
  • Actively promote and support a Culture of Care, ensuring improvement work enhances compassionate practice, dignity, relational working and the overall therapeutic environment.
  • Support staff to adopt behaviours associated with compassionate leadership, including kindness, listening, psychological safety and openness.
  • Ensure patient and carer voice informs all improvement work through co‑production and genuine engagement.
  • Reinforce Trust values in daily improvement activities, modelling high‑quality interpersonal behaviours.

Qualifications

  • Degree‑level education (or equivalent) in a relevant field such as healthcare, business or Quality Improvement.
  • Training in Quality Improvement or a similar discipline.

Experience

  • Experience supporting change management or Lean projects, facilitation, multidisciplinary team working, and data analysis.
  • Experience working in Mental Health care services.

Knowledge

  • Lean methodology, improvement science and mental health pathways and flow principles.
  • Advanced knowledge of mental health system pressures.
