We have an exciting opportunity in the Northumberland 0 – 19 Service working within our Health Visiting Teams for a Family Health Practitioner.
The successful applicants will be based in Hexham working within our teams to deliver care across the locality.

This post is for 30 hours per week
Main duties of the job
The Family Health Practitioner is a member of the 0-19 Integrated Public Health Service (IPHS), working within the policies and procedures relating to the 0 – 19 service.
The post holder will work closely with the SCPHN within a skill mixed team to support the delivery of the 0-19 Service Specification and Healthy Child Programme (HCP).
The aim of the service is to provide universal access to children, young people and families to the Healthy Child Programme and facilitate early identification of additional and or complex needs.
The post holder will contribute to the Public Health Agenda, to support the promotion of health and reduction in health inequalities. They will work in partnership with key partners and agencies to meet local and national agendas

Criteria
- NVQ Level 3 Qualification or equivalent.
- Experience of working with and supporting parents and families.
- Ability to communicate with a variety of audiences both written and verbally.
- Knowledge of child/adolescent development.
- Ability to relate to people of different backgrounds.
- Working with a range of statutory and voluntary organisations.
- Knowledge of the Solihull Approach model.
- Knowledge of parenting programmes.
