Key Responsibilities
- Provide strategic and operational management to Camden Rapid Access Service: Discharge to assess, Urgent community response and Hospital at home.
- Provide transformational, integrated leadership and service modernisation to the teams.
- Enhance the development of evidence‑based clinical practice and Allied Health Professional roles.
- Provide clinical and professional leadership to the team.
- Be aware of national and local agendas for intermediate care services and Allied Health Professions.
- Work with the Rapid Access Service Operational Lead and wider health service stakeholders to develop strategies in line with the National Agenda around admission avoidance and supported discharge.
- Have oversight of the work of the Therapy Team.
- Work with the AHP Professional Leads for CNWL to develop Therapy roles within CNWL and promote the professions.
Clinical Duties
- Develop and contribute to the development of clinical pathways in conjunction with the ICB and other stakeholders and providers.
- Promote an integrated working culture across Therapy, Medical and Nursing teams to enhance rehabilitation outcomes and the patient experience.
- Support the team in the implementation of treatment plans to ensure patients receive a high quality of care at all times, liaising with partnership organisations to risk‑manage complex cases.
- Be actively involved in the design and implementation of research and/or clinical audits/trials to evaluate aspects of the service and identify areas for development, maintenance and improvement.
- Develop and maintain good working relationships and liaison with colleagues and other health professionals regarding patient care, e.g. GPs, LAS and Acute Trust colleagues.
- Monitor outcome‑measure data sets to determine service performance and capacity of complexity.
- Provide oversight monitoring of team capacity, allocation and waiting times to ensure safe service provision and flow across the health and social care system.
Person Specification
Education and Qualification (Essential)
- Current HCPC Registration as a physiotherapist or occupational therapist.
- Bachelor’s Degree (BSc).
- Evidence of continued professional development.
- Evidence of working in a multidisciplinary team.
- Practice supervisor training.
Education and Qualification (Desirable)
- Masters degree or willingness to work towards one.
- Leadership/management course.
Experience (Essential)
- Substantial previous experience working in a community admission avoidance service.
- Evidence of working within the MDT.
- Understanding of the role of community teams in admission prevention and supporting timely discharge.
Experience (Desirable)
- Innovative and flexible working.
- Experience in an acute setting.
Communication (Essential)
- Able to communicate with the multidisciplinary team, present cases, take note of actions, update others in the team and use handover effectively. Speaking face to face but also by telephone and writing letters and reports.
- Maintaining accurate, timely, concise and legible clinical records both electronic and handwritten which adhere to the CNWL policies.
- Able to talk to people about sensitive issues in a supportive manner but also challenge others assertively where necessary.
Communication (Desirable)
- Understanding of PSIRF processes in relation to patient safety incidents.
Skills and Knowledge (Essential)
- Maintaining accurate, timely, concise and legible clinical records both electronic and handwritten which adhere to the CNWL policies.

- Able to maintain electronic patient records.
- Use Microsoft Word/Excel/PowerPoint for letters, reports and presentations.
