Job Description:

An exciting opportunity has arisen within the Heywood & Middleton Community Mental Health Team for a Community Mental Health Nurse. The role is to provide mental health care to people of working age who have moderate to severe mental health problems across a wide range of presenting conditions.

Responsibilities

  • Act as a Community Mental Health Nurse for a defined caseload working with evidence‑based interventions.
  • Assess and review needs.
  • Formulate care plans.
  • Implement care plans.
  • Assess, monitor and manage risk.
  • Monitor, review and evaluate care.
  • Liaise with all agencies and members of the multi‑disciplinary team.
  • Carry out clinic duties such as the administration of Clozaril / depot injections either in the client’s home or in the clinics if nurse qualified where required.
  • Maintain accurate, up‑to‑date electronic records.
  • Participate in the CMHT’s duty system on a rota basis.
  • Offer a rapid response to service users’ immediate needs, including those arising from crisis.
  • Work in a therapeutically informed way within the multi‑disciplinary team to address the needs of service users with complex emotional disorders.
  • Maintain persistent engagement where there are complex mental health difficulties.
  • Prioritise and work flexibly with the MDT in order to respond to rapidly changing clinical needs.
  • Promote the physical health and wellbeing of service users.

Requirements

  • RMN: Registered Mental Health Nurse.
  • Experience in a relevant area of mental health practice.
  • Current driving licence or supported driver with use of vehicle.
