A vacancy at West Hertfordshire Teaching Hospitals NHS Trust. We are looking for an Advanced Clinical Practitioner for a rotational post to support the General Medicine virtual ward within our Virtual Hospital partnership between CLCH and West Herts Virtual Hospital.
Key Details
The post will rotate between the partnership organisations, including West Hertfordshire, Watford General site, and Central London Community Hospital Trust. The role requires travel to all sites and to patient addresses within our geographical catchment area. Working pattern is 8am‑8pm shifts, with one in four weekends.
Responsibilities
- Act as a key Clinical Lead within service delivery and governance; drive the successful management of care for patients identified for the virtual ward environment.
- Work autonomously delivering independent assessment, clinical leadership, supervision, and mentorship for multidisciplinary professionals.
- Collaborate with all members of the multidisciplinary team to facilitate admission avoidance and early supported discharge.
- Promote education and facilitate evidence‑based practice, taking a lead on clinical audit, research and service development.
- Provide expert advice for clinical practice, leadership, supervision, and support to other ACPs, junior medical staff and clinical nursing staff of all grades.
- Demonstrate advanced expertise, specialist knowledge, and high levels of judgement, discretion and decision‑making in clinical care.
- Work within the four clinical pillars of advanced practice – clinical practice, facilitating learning, leadership and evidence, research and development – and demonstrate this within your portfolio.
- Support the operational development of the service and the delivery of advanced clinical practice, professional leadership, education, training and service development, research and evaluation.
Qualifications
We welcome applications from experienced Band 7 practitioners and are committed to supporting progression to Band 8a upon successful completion of all required competencies.
The post will be structured around the four pillars of advanced clinical practice as set out in the multi‑professional consultant level practice capability and impact framework: expert clinical practice, professional leadership and consultancy function, education, training and development, research, evaluation and service development across the system.
